Medicare Advantage Cost Components
- Monthly Premium: Ranges from $0 to $100+/month. Many plans are $0 premium but you still pay the Medicare Part B premium (~$185/mo in 2026).
- Deductible: Your annual payment before insurance kicks in. Many MA plans have $0 medical deductible. Drug deductibles are separate.
- Copays: Fixed amounts you pay per service (e.g., $10 for primary care, $40 for specialist).
- Coinsurance: A percentage of the cost you pay (e.g., 20% for hospital stays after deductible).
- Out-of-Pocket Maximum (MOOP): The most you’ll pay in a year for covered in-network services. The 2026 cap is $9,350 for in-network and $13,300 for combined in/out-of-network PPO plans.
HMO vs PPO Cost Differences
- HMO plans: Lower premiums and copays. In-network only (except emergencies).
- PPO plans: Higher premiums. Out-of-network care covered but at higher cost.
Estimating Your Annual Total Cost
Formula: (Monthly Premium × 12) + Estimated Copays + Drug Costs = True annual cost. For healthy enrollees, a $0-premium plan with low copays is usually best. For those with frequent care needs, compare the annual premium + likely copay totals against Gold-equivalent plans.
